They've spealt it Kow Otani, I've seen it Kow Ohtani and I thought it was Koh Ohtani!??

All depends upon which romanization style you use...
The 'oh'/'ow's are just a way to show that the O sound is supposed to be lengthened. Sometimes people use 'oh', 'ou', 'ow'...and even just 'o'.
(From my experience 'ow' is a lot less common than the rest...)
Use whichever you feel most comfortable with (or whichever one he decides to use as his official romanization)...
